AB786-AA1,1,2 1. Page 1, line 1: delete "report on".

AB786-AA1,1,4 2. Page 1, line 1: delete the material beginning with "and" and ending with "dementia" on line 2 and substitute "proposal".

AB786-AA1,1,6 3. Page 2, line 4: delete the material beginning with "report" and ending with "program" on line 6 and substitute "proposal for one or more pilot programs".

AB786-AA1,1,7 4. Page 2, line 7: delete "report" and substitute "proposal".
