Services available. The children with special health care needs program may assist in:

(1) Providing for medical examination, consultation and treatment.

(2) Providing for audiological evaluation and consultation.

(3) Providing for assistance in referral for educational services.
DHS 143.02(5)
(5) Providing for referral for social work services and consultation.

DHS 143.03(1)(1) Payment may be made for hearing assessment and hearing aid evaluation for medically indigent families to a clinical audiologist who is on the CSHCN program list of participating clinical audiologists.

(2) Payment shall be made for hearing aids (all or in part) for medically indigent families. Fitting shall be approved by a clinical audiologist who is on the program list of participating clinical audiologists.

Administrative procedures. For the administration of medical care and audiological assessment:
DHS 143.04(1)
(1) The results and recommendations of the physician's examination shall be forwarded to the CSHCN program clinical audiologist.
DHS 143.04(2)
(2) The results and recommendations of the assessment by the clinical audiologist shall be forwarded to the CSHCN program clinical audiologist.
DHS 143.04(3)
(3) The results and recommendations of the assessment by the clinical audiologist, including pertinent information regarding the fitting of the hearing aid, shall also be shared with the hearing aid specialist.

(4) The parent of the child shall designate the hearing aid specialist who will fit the hearing aid.
DHS 143.04(5)
(5) The hearing aid specialist shall be licensed to dispense hearing aids in the state of Wisconsin. The hearing aid specialist shall also follow the rules for financial involvement in the purchase of hearing aids.

(6) If the CSHCN program pays the full cost of the hearing aid or accessory, the designated hearing aid specialist shall not charge any additional amount. In those cases where a percentage of the cost for any hearing aid or accessory is paid by the CSHCN program, the designated hearing aid specialist shall only charge the recipient of services the remaining amount of the standard fee determined by the CSHCN program.

(7) The parent receiving aid for payment of services shall have the child return to the clinical audiologist for a final check of the hearing aid before any monies are paid. In the event the parent is unwilling or unable to return the child to the clinical audiologist following the fitting of the hearing aid, consultation with the CSHCN program audiologist shall be required.

(8) The hearing aid specialist shall confer with the clinical audiologist concerning any modification in the fitting of the hearing aid.

(9) A financial eligibility form shall be submitted to the CSHSN program and written approval shall be received by the parent prior to rendering of and payment for any services.

(10) Reimbursement shall be made only to the provider of care and not to the recipient of service. Where there is medical insurance coverage, the provider shall submit the claim and the CSHCN program will pay the amount not covered by insurance.

Requirements for participating clinical audiologists. DHS 143.05(1)(a)
(a) Shall complete graduate studies in audiology leading to a master's and/or higher graduate degree.
DHS 143.05(1)(b)
(b) Shall submit evidence of having completed 300 hours of supervised clinical experience and 60 hours of course work in audiology and related areas.
DHS 143.05(1)(c)
(c) Shall complete an additional year of full-time supervised clinical internship in audiology or be in the process of completing this requirement.
DHS 143.05(1)(d)
(d) Shall pass the National Examination in Audiology administered by the educational testing service, Princeton, New Jersey, or who have received a Certificate of Clinical Competence in Audiology.

(2) Persons wishing to be included on the state agencies' list of participating clinical audiologists must submit evidence that the above requirements have been met. Those clinical audiologists who are in the process of meeting the certification standards outlined above will be reviewed individually for participation in this program. Any other person not meeting these requirements may present his/her qualifications to the CSHCN program audiologists for review and consideration.

(3) Membership in any professional association is not required.

(4) All information pertaining to the above requirements shall be submitted in writing to CSHCN program clinical audiologists.

Procedures for participating clinical audiologists. DHS 143.06(1)(1) In every case where the CSHCN program is involved in payment, a complete report shall be made to the CSHCN program within 2 weeks of completion of the evaluation.

(2) In all cases where audiometric evaluation has indicated abnormal hearing, arrangements shall be made for an examination of the ear by a physician specializing in diseases of the ear.

(3) A complete report shall be submitted to the CSHCN program clinical audiologist. It shall include all pertinent information and a complete audiogram. Educational programming recommendations from the audiologists are desired but specific educational placement shall be the responsibility of the local educational agency.

Procedures for clinical audiologists dispensing hearing aids. DHS 143.07(1)
(1) Shall meet the CSHCN program requirements for participating clinical audiologists in order to qualify for reimbursement for services rendered.

(2) Any reference to state agencies in advertising or promotion of any kind is prohibited.

(3) Facilities utilized in evaluating the recipient of CSHCN program services shall meet the approval of the CSHCN program.

DHS 143.08(1)(1) Participating physicians are licensed physicians who have demonstrated their competence and are certified by the American Board of Otolaryngology, American Academy of Ophthalmology and Otolaryngology, and some others as determined by the CSHCN program.

(2) Participating clinical audiologists are those clinical audiologists who have met the requirements of the CSHCN program.
DHS 143.08(3)
(3) Participating clinical audiologists dispensing hearing aids are those clinical audiologists who have met the requirements of the CSHCN program. They shall be licensed in the state of Wisconsin to dispense, fit and service hearing aids.
DHS 143.08(4)
(4) Children with special health care needs program clinical audiologists are those clinical audiologists employed by the department of health services, children with special health care needs program, to administer the Wisconsin state hearing conservation program.

(5) Participating hearing aid specialists are hearing aid specialists licensed in the state of Wisconsin who have agreed to supply hearing aids and services pursuant to the above rules.

(6) The program for children with special health care needs' list of participating physicians, clinical audiologists, and hearing aid specialists are those physicians, clinical audiologists and hearing aid specialists who have agreed to work with the CSHCN program pursuant to these rules.
