PSC 166.01(1)(a)(a) After the effective date of this order, any telephone utility of this state may present to this commission a statement, accompanied by an accurate map, indicating the territory in any town or towns of this state in which said telephone utility presently proposes, subject to its filed extension rules, to make extensions of its lines and service at any time when such service may be requested or demanded by any person within the territory as thus designated.

(b) Proof of service of such statement and map upon all other telephone utilities furnishing service in the town or towns designated in such statement shall be furnished with the presentation thereof.

(c) Objections by such other utilities to the proposed extensions of lines or service indicated by such statement and map shall be filed with the commission within 20 days from the date of service of such statement upon any of such other utilities.

(d) Upon presentation of such statement and accompanying map to the commission, and proof of service of the same as above required, the commission will consider the same and, unless the commission within 60 days after such presentation of said statement and map shall consider and find that the proposed extensions of lines or service, or some part thereof, as indicated therein is not required by public convenience and necessity, then the said statement and accompanying map shall be filed, and such filing shall be considered as a full and complete compliance by the utility filing the same with the requirements of s.
196.50 (2), Stats.

(2) Determination by commission. If no objection to the extension of lines or service as indicated on any statement and map is filed within the time as above prescribed, the commission may upon due consideration and within the 60-day period as above provided, direct that such statement and map be accepted for filing; and such filing, pursuant to such direction, shall constitute a determination by the commission that it does not find that the extensions of lines or service, or both, as proposed in said statement and map are not required by public convenience and necessity.

(3) Filing not determinative. Neither the presentation nor the filing of any statement and map pursuant to the provisions of this order shall be determinative of the restriction of the undertaking of service of the utility presenting the same to service within the territory designated in such statement and map, nor shall it be construed as limiting any statutory power of the commission.

Conforming maps. Every exchange area boundary map filed with the commission shall conform to the rules in this chapter. Exhibit A attached is a specimen map which conforms to all of the requirements established in this chapter.

Maps kept available. In each exchange office of the filing utility to which the public shall have access there shall be kept available to the public an exchange area boundary map for that exchange which shall be the same as the map which is on file with the commission.

Map, requirements. The printed matter on the map shall show:

(1) On the upper left-hand corner the exact name of the utility on the first line, the name of the exchange on the second line, and the names of the counties shown on the map on the third line.

(2) On the upper right-hand corner the P.S.C. of W. No. on the first line, the Sheet No. on the second line and the Amendment No. on the third line.

(3) On the lower left-hand corner shall be shown the code and/or symbols conforming with
s. PSC 166.05.

(4) On the lower right-hand corner shall be shown the method of P.S.C.W. acceptance on the second line, the date on the third line and the date that the map is effective on the fourth line.

Code. The following code and/or symbols shall be used on each map.

Changes. When changes are necessary, a new map incorporating such changes shall be filed and designated as a revised sheet. For example, the original map will bear the designation "Original Sheet No. ", the revised map "First Revised Sheet No.".

Accompanying statement. All maps filed with the commission shall be accompanied by a statement of undertaking to serve in the following form:
"The attached map, dated , delineates the boundary and territory of the exchange in which the Telephone Company now serves and will serve the public. The Telephone Company undertakes, subject to its extension rules and filed tariffs, to make extensions of its lines and service at any time when such service may be requested or demanded by any person located within such territory as designated on such map.
The Telephone Company will not extend its lines and service beyond the territory and exchange boundary defined on the attached map, except:
(a) to serve persons located in the territory designated as open on such map.
(b) to continue service to customers presently located in foreign exchange territory as indicated on such map.

Service of map and statement. PSC 166.08(1)
(1) Service of the map and statement upon other telephone utilities furnishing service in the town or towns designated on such map should be made by addressing a letter to such utilities, enclosing a copy of the map and statement. The return of a copy of such letter, acknowledging receipt of the map and statement, shall be sufficient to constitute service thereof upon such utilities within the terms of the order.

(2) When a revision in the map is made, the service of the map and statement is required to be made only upon those telephone utilities which are operating in the town or towns in which the boundary line is changed by such revision. The other telephone utilities which may have theretofore been served with the original map and statement shall be furnished a copy of the revised map.

Letter of transmittal. All maps and statements filed with the public service commission shall be accompanied by a letter of transmittal indicating which of the exchange area boundary lines shown are common boundaries with adjoining utilities, the names of such other utilities, and, if any, the names of other utilities furnishing service in the town or towns shown on the map. Proof of service of copies of the map and statement on such companies pursuant to
s. PSC 166.08 should be attached.
