SB1-SA8,1,3 1. Page 1, line 12: delete the material beginning with "confidentiality" and ending with "hospitals;" on line 14.

SB1-SA8,1,5 2. Page 9, line 1: delete the material beginning with that line and ending with page 13, line 17.

SB1-SA8,1,6 3. Page 27, line 3: delete lines 3 to 18.

SB1-SA8,1,7 4. Page 30, line 11: delete lines 11 to 20.
