SB85-SA1,1,2 1. Page 2, line 11: after "provider, the" insert "price that the".

SB85-SA1,1,4 2. Page 2, line 12: delete that line and substitute "provider stated for the service before the service was provided.".

SB85-SA1,1,5 3. Page 2, line 17: after "service," insert "absconds and".
