SB451-SA2,1,11 At the locations indicated, amend the bill as follows:
SB451-SA2,1,3 21. Page 2, line 13: after "programs," insert "copayments, deductibles, or

SB451-SA2,1,4 42. Page 4, line 8: after that line insert:

SB451-SA2,3,9 9632.867 Oral and injected chemotherapy. (1) Definitions. In this section:
SB451-SA2,3,1210 (a) "Chemotherapy" means drugs and biologics that kill cancer cells directly,
11including antineoplastics, biologic response modifiers, hormone therapy, and
12monoclonal antibodies, and that are used to do any of the following:
SB451-SA2,3,1313 1. Cure a specific cancer.
SB451-SA2,3,1414 2. Control tumor growth when cure is not possible.
SB451-SA2,3,1515 3. Shrink tumors before surgery or radiation therapy.
SB451-SA2,3,1716 4. Destroy microscopic cancer cells that may be present after a tumor is
17removed by surgery to prevent a cancer recurrence.
SB451-SA2,3,1818 (b) "Disability insurance policy" has the meaning given in s. 632.895 (1) (a).
SB451-SA2,3,1919 (c) "Self-insured health plan" has the meaning given in s. 632.85 (1) (c).
SB451-SA2,4,2 20(2) Copayment, deductible, or coinsurance requirements; limitations. (a) A
21disability insurance policy that covers injected or intravenous chemotherapy and
22oral chemotherapy, or a self-insured health plan that covers injected or intravenous
23chemotherapy and oral chemotherapy, may not require a higher copayment,
24deductible, or coinsurance amount for oral chemotherapy than it requires for

1injected or intravenous chemotherapy, regardless of the formulation or benefit
2category determination by the policy or plan.
SB451-SA2,4,63 (b) A disability insurance policy or a self-insured health plan may not comply
4with par. (a) by increasing the copayment, deductible, or coinsurance amount
5required for injected or intravenous chemotherapy that is covered under the policy
6or plan.".

SB451-SA2,4,11 9(1c) The treatment of sections 40.51 (8) and (8m), 66.0137 (4), 120.13 (2) (g),
10185.983 (1) (intro.), 609.837, and 632.867 of the statutes first applies to all of the
11following:
SB451-SA2,4,1512 (a) Except as provided in paragraphs (b) and (c), disability insurance policies
13that are issued or renewed, and governmental or school district self-insured health
14plans that are established, extended, modified, or renewed, on the effective date of
15this paragraph.
SB451-SA2,4,1816 (b) Disability insurance policies covering employees who are affected by a
17collective bargaining agreement containing provisions inconsistent with this act
18that are issued or renewed on the earlier of the following:
SB451-SA2,4,1919 1. The day on which the collective bargaining agreement expires.
SB451-SA2,4,2120 2. The day on which the collective bargaining agreement is extended, modified,
21or renewed.
SB451-SA2,5,222 (c) Governmental or school district self-insured health plans covering
23employees who are affected by a collective bargaining agreement containing

1provisions inconsistent with this act that are established, extended, modified, or
2renewed on the earlier of the following:
SB451-SA2,5,33 1. The day on which the collective bargaining agreement expires.
SB451-SA2,5,54 2. The day on which the collective bargaining agreement is extended, modified,
5or renewed.".
SB451-SA2,5,6 67. Page 59, line 19: after that line insert:
SB451-SA2,5,9 7"(1c) Chemotherapy. The treatment of sections 40.51 (8m), 66.0137 (4), 120.13
8(2) (g), 185.983 (1) (intro.), and 609.837 of the statutes and Section 118n (1c) of this
9act take effect on the day after publication.
SB451-SA2,5,1210 (1d) Oral and injected chemotherapy. The treatment of section 632.867 (2) (a)
11of the statutes takes effect on the first day of the 7th month beginning after
12publication.".
