AB518-AA1,1,2 1. Page 2, line 3: before “request" insert “written".

AB518-AA1,1,3 2. Page 2, line 8: delete “$30" and substitute “$50".

AB518-AA1,1,5 3. Page 4, line 16: delete the material beginning with “plus $500" and ending with line 19 and substitute “or $350, whichever is less.”.
