AB399-AA4,1,1 At the locations indicated, amend the bill as follows:

AB399-AA4,1,2 1. Page 7, line 8: delete that line and substitute: "relief medication.

AB399-AA4,1,3 3. If you feel that you cannot comply with this document, you must".

AB399-AA4,1,5 2. Page 7, line 10: after the underscored period insert: "Refusal or failure to make a good faith attempt to do so constitutes unprofessional conduct.".

AB399-AA4,1,6 3. Page 7, line 11: delete " 3." and substitute "4.".

AB399-AA4,1,8 4. Page 7, line 22: delete the material beginning with that line and ending with page 8, line 3.
