AB399-AA5,1,1 At the locations indicated, amend the bill as follows:

AB399-AA5,1,4 1. Page 4, line 16: on lines 16 and 17, delete "as determined by 2 physicians who have personally examined me," and substitute "as determined by 2 physicians who have personally examined me,".

AB399-AA5,1,7 2. Page 5, line 1: on lines 1 and 2, delete "as determined by 2 physicians who have personally examined me," and substitute "as determined by 2 physicians who have personally examined me,".

AB399-AA5,1,9 3. Page 5, line 12: after the underscored comma insert "as determined by 2 physicians who have personally examined me,".
