AB441-ASA1-AA33,1,3 1. Page 6, line 15: after "orally" insert: ", in writing or by other accurate and complete means that are appropriate to a woman's circumstances".

AB441-ASA1-AA33,1,5 2. Page 6, line 19: delete "The" and substitute: "If he or she has not already done so, the".
