AB441-SA17,1,1 At the locations indicated, amend the engrossed bill as follows:

AB441-SA17,1,2 1. Page 10, line 11: after "exists" insert: "and except as provided in par. (h)".

AB441-SA17,1,4 2. Page 14, line 8: after the period insert: "If par. (h) applies, the woman shall certify on the form as required under that paragraph.".

AB441-SA17,1,5 3. Page 18, line 8: after that line insert:

AB441-SA17,1,9 "(h) Waiver. The requirements under par. (c) 1. (intro.) and 2. (intro.) as to the provision of information are waived if the woman certifies on the form that is specified under par. (c) 5. that she does not require the information that is specified under par. (c).".
