AB693-ASA1-AA4,1,1 At the locations indicated, amend the substitute amendment as follows:

AB693-ASA1-AA4,1,2 1. Page 1, line 9: delete lines 9 to 13 and substitute:

AB693-ASA1-AA4,1,7 "(2) Consent required. (a) No official, employe or agent of a school board may conduct any written survey or questionnaire of pupils that may reveal information about any of the following without the written consent of the pupil, if the pupil is an adult or an emancipated minor, or without the written consent of the pupil's parent or guardian, if the pupil is an unemancipated minor:".

AB693-ASA1-AA4,1,8 2. Page 2, line 15: delete lines 15 to 25 and substitute:

AB693-ASA1-AA4,1,13 "(b) An official, employe or agent of a school board shall obtain written consent under par. (a) for each survey or questionnaire conducted. In seeking consent, the official, employe or agent shall explicitly describe, in writing, the specific survey or questionnaire to which the consent will apply. The request for consent shall include a form for the purpose of granting consent.".
