AB100-ASA1-AA14,1,3 3" Section 3009t. 146.83 (2) of the statutes is amended to read:
AB100-ASA1-AA14,1,84 146.83 (2) The health care provider shall provide each patient with a statement
5paraphrasing the provisions of this section and specifying the procedures by which
6the health care provider protects the confidentiality of patient health care records

7either upon admission to an inpatient health care facility, as defined in s. 50.135 (1),
8or upon the first provision of services by the health care provider.

AB100-ASA1-AA14,2,2 10146.832 Use of social security number to identify a patient. No one may
11use the social security number of a patient to identify that patient or his or her

2services that are funded by state or federal funds.
