(2r) (a) Any birth parent whose parental rights to his or her child have not
4been terminated in this state at any time, or who did not consent to the adoption of
5his or her child in this state before February 1, 1982, may file with the department
6or the agency contracted with under sub. (11) an affidavit authorizing the
7department or agency to provide any available information about the identity and
8location of that child to any person who requests that information and who meets all
9of the following criteria:

1. The person is a birth child of the birth parent and the parental rights of the
11birth parent to that person have been terminated in this state at any time, or the
1birth parent has consented to the adoption of that person in this state before
2February 1, 1982.

2. The person is a brother or sister by birth or a half brother or half sister by
4birth of the child.

3. The person is 21 years of age or over at the time of the request.

(b) An affidavit filed under par. (a) may be revoked at any time by notifying the
7department or agency in writing.".
