SENATE AMENDMENT 3, TO SENATE SUBSTITUTE AMENDMENT 1, TO 1997 SENATE BILL 315

SB315-SSA1-SA3,1,6 3. Page 7, line 6: after that line insert:

SB315-SSA1-SA3,1,7 " Section 21m. 153.05 (13) of the statutes is created to read:

SB315-SSA1-SA3,2,2 153.05 (13) The department may waive the requirement under sub. (1), (5) or (8) for a physician, as defined in s. 448.01 (5), who requests the waiver and presents evidence to the department that the requirement under sub. (1), (5) or (8) is burdensome, under standards established by the department by rule. The department shall develop a form for use by a physician in submitting a request under this subsection.".
