SB494-AA2,1,2 At the locations indicated, amend the bill, as shown by senate substitute amendment 2, as follows:

SB494-AA2,1,4 1. Page 11, line 22: delete "The" and substitute "Except as provided in par. (e), the".

SB494-AA2,1,5 2. Page 12, line 21: after "(2)" insert "and except as provided in par. (e)".

SB494-AA2,1,6 3. Page 12, line 23: after that line insert:

SB494-AA2,1,12 "(e) Refusal for religious reasons. The department may not require an applicant for an approval specified in par. (a) 1. to 3. to provide his or her social security number if the applicant refuses to provide his or her social security number for religious reasons. The department may not deny an application to issue or renew an approval on the basis of par. (a) if the applicant for or holder of the approval refuses to provide his or her social security number for religious reasons.".
