Page 5, line 9: after "coinsurance." insert "This paragraph applies to a health maintenance organization, as defined in s. 609.01 (2), or a preferred provider plan, as defined in s. 609.01 (4), only with respect to preventive pediatric health care services provided by selected providers, as defined in s. 609.01 (6), of the health maintenance organization or preferred provider plan.".
