AB474-ASA1-AA10,1,5 2. Page 2, line 1: delete lines 1 and 2 and substitute "of the pupil's parent or guardian,".

AB474-ASA1-AA10,1,6 3. Page 2, line 16: delete "written".

AB474-ASA1-AA10,1,8 4. Page 2, line 22: delete the material beginning with that line and ending with page 3, line 2.
