AB474-ASA1-AA4,1,6 4. Page 2, line 17: delete the material beginning with "The official" and ending with "questionnaire." on line 19.

AB474-ASA1-AA4,1,9 5. Page 2, line 21: after "apply." insert "Consent shall be considered to have been given under par. (b) if the pupil or the pupil's parent or guardian fails to respond to the written notice before the date of the survey or questionnaire.".
