AB474-ASA1-AA9,1,1 At the locations indicated, amend the substitute amendment as follows:

AB474-ASA1-AA9,1,6 1. Page 1, line 12: delete the material beginning with "the written" and ending with "minor" on page 2, line 2, and substitute "first notifying the pupil, if the pupil is an adult or an emancipated minor, or the pupil's parent or guardian, if the pupil is an unemancipated minor, of the nature of the survey or questionnaire and the right of the pupil to refuse to participate in the survey or questionnaire under par. (d)".

AB474-ASA1-AA9,1,7 2. Page 2, line 16: delete lines 16 to 21.

AB474-ASA1-AA9,1,10 3. Page 2, line 24: delete the material beginning with "if" and ending with "questionnaire" on page 3, line 2, and substitute "unless the primary purpose of the survey or questionnaire is academic".
