AB795-ASA1-AA1,1,1 At the locations indicated, amend the substitute amendment as follows:

AB795-ASA1-AA1,1,2 1. Page 3, line 19: delete "par. (b)" and substitute "pars. (b) and (bm)".

AB795-ASA1-AA1,1,3 2. Page 4, line 8: after that line insert:

AB795-ASA1-AA1,1,6 "(bm) Jail medical staff need not complete the form if the jailer or his or her designee provides a copy of the prisoner's complete medical file to the receiving institution intake staff at the time of the transfer.".
