SB182-SA1,1,11 At the locations indicated, amend the bill as follows:
SB182-SA1,1,2 21. Page 4, line 2: delete lines 2 and 3 and substitute:
SB182-SA1,1,3 3"632.895 (14) Contraceptive articles and services. (a) In this subsection:
SB182-SA1,1,54 1. "Contraceptive article" means any drug, medicine, mixture, preparation,
5instrument,".
SB182-SA1,1,6 62. Page 4, line 11: after that line insert:
SB182-SA1,1,8 7"2. Religious employer" means an entity that satisfies all of the following
8criteria:
SB182-SA1,1,99 a. The inculcation of religious values is the purpose of the entity.
SB182-SA1,1,1110 b. The entity employs primarily persons who share the religious tenets of the
11entity.
SB182-SA1,1,1312 c. The entity serves primarily persons who share the religious tenets of the
13entity.

1d. The entity is exempt from filing a federal annual information return under
2section 6033 (a) (2) (A) (i) and (iii) and (C) (i) of the Internal Revenue Code.".
SB182-SA1,2,3 33. Page 5, line 5: after that line insert:
SB182-SA1,2,11 4"4. A disability insurance policy that is issued to a religious employer, if the
5religious employer requests that the insurer issuing the policy not provide the
6coverage specified in par. (b) on the basis that the articles and services covered are
7contrary to the religious employer's religious tenets. A religious employer that
8makes a request under this subdivision shall provide written notice to a prospective
9insured under the policy, prior to that person's coverage under the policy, that
10specifies the articles and services under par. (b) that will not be covered on the basis
11of the employer's request.".
