4applicable exchanges meets thresholds established in rules promulgated by the
5commission, and after determining the rate increments necessary to support the
6extended area service, the commission shall require the affected telecommunications
7utilities to conduct a survey of customer willingness to pay the rate increments. The
8survey shall be approved by the commission and distributed to all customers who
9may experience a rate change if the petition were granted.".
