3department shall distribute the forms to any sheriff who issues licenses under sub.
4(2) (a) or (c) for distribution to applicants. The form shall request all of the following:

9list in subd. 2. or with any common nonprescription drug product, as defined in s.
10450.01 (13m).
SB214-SSA2-SA52,2,1411 4. A physician statement as to whether the list in subd. 2. contains any
12prescription drug that could prevent him or her from safely handling a handgun or
13that is prescribed for any mental illness or for a physical illness that could prevent
14him or her from safely handling a handgun.".
