116.81 Corroboration in lieu of presentation of identification card. 12Whenever under this chapter an elector who votes in person is required to present
1an operator's license issued to the elector under ch. 343, an identification card issued
2to the elector by a U.S. uniformed service, or an identification card issued to the
3elector under s. 343.50 in order to register to vote or to vote, the elector may vote or
4register to vote without presenting a license or identification card if the elector
5presents acceptable proof of residence under s. 6.55 (7) and another corroborating
6elector who resides in the same municipality as the elector presents the required
7license or identification card and that corroborating elector swears, on a form
8prescribed by the board for this purpose, that the corroborating elector knows that
9the elector is an elector of the municipality and that the corroborating elector has not
10corroborated the qualifications of more than 2 electors at the same election at which
11the corroboration is made.".

(1) (c) Prescribe forms required by ss. 6.24 (3) and (4), 6.30 (4), 6.33 (1),
196.40 (1) (a), 6.47 (1) (a) 2. and (3), 6.55 (2) and (3),
6.81, and 6.86 (2) and (3). All such
20forms shall contain a statement of the penalty applicable to false or fraudulent
21registration or voting through use of the form. Forms are not required to be furnished
22by the board.".
