16.81 Corroboration in lieu of presentation of identification card. 2Whenever under this chapter an elector who votes in person is required to present
3an operator's license issued to the elector under ch. 343, an identification card issued
4to the elector by a U.S. uniformed service, or an identification card issued to the
5elector under s. 343.50 in order to register to vote or to vote, the elector may vote or
6register to vote without presenting a license or identification card if the elector
7presents acceptable proof of residence under s. 6.55 (7) and another corroborating
8elector who resides in the same municipality as the elector presents the required
9license or identification card and that corroborating elector swears, on a form
10prescribed by the board for this purpose, that the corroborating elector knows that
11the elector is an elector of the municipality and that the corroborating elector has not
12corroborated the qualifications of more than 2 electors at the same election at which
13the corroboration is made.".

(1) (c) Prescribe forms required by ss. 6.24 (3) and (4), 6.30 (4), 6.33 (1),
216.40 (1) (a), 6.47 (1) (a) 2. and (3), 6.55 (2) and (3),
6.81, and 6.86 (2) and (3). All such
22forms shall contain a statement of the penalty applicable to false or fraudulent
23registration or voting through use of the form. Forms are not required to be furnished
24by the board.".
