AB305-AA5,1,3 2. Page 4, line 23: delete lines 23 to 25.

AB305-AA5,1,6 3. Page 12, line 5: delete "Unless any of the following is" and substitute "If any of the following has a direct professional relationship with or provides professional services directly to the declarant and is not".

AB305-AA5,1,7 4. Page 12, line 13: delete that line.

AB305-AA5,1,8 5. Page 12, line 15: delete that line.
