SENATE AMENDMENT 1, TO 2011 SENATE BILL 450

SB450-SA1,1,1 At the locations indicated, amend the bill as follows:

SB450-SA1,1,11 1. Page 2, line 8: delete lines 8 to 11 and substitute "(2) Development of survey form. The department, the department of health services, and the department of workforce development jointly shall submit a plan within 12 months after the effective date of this subsection .... [LRB inserts date], to the joint committee on finance to develop one or more survey forms to gather data under ss. 447.05 (2), 448.07 (1m), 448.55 (4), 450.08 (3), 455.06 (2), and 457.20 (5). After a survey form is developed, the department, the department of health services, and the department of workforce development jointly may revise the form as those agencies deem appropriate. Any data gathered under s. 447.05 (2), 448.07 (1m), 448.55 (4), 450.08 (3), 455.06 (2), or 457.20 (5)".
