(5) (i) 2. An insurer writing motor vehicle insurance policies shall
5provide notice of the availability of a provision permitted under subd. 1. and an
6explanation of the effect of the provision to any person who applies for insurance
7under such a policy on or after the effective date of this subdivision .... [LRB inserts
8date], and to one insured under such a policy written by the insurer that is in effect

10the notice of the first renewal of that policy on or after the effective date of this

(5) (i) 3. The notice and explanation required under subd. 2. must be in
3writing and must include a line or box, or another space, for the applicant or insured
4to check or otherwise indicate that he or she was informed about, and understands
5the effect of, the provision and either agrees or does not agree to include the provision
6in the policy. The notice and explanation need not be on a separate form, but may
7be a part of the policy, and may be made available online. An insurer is required to
8provide the notice and explanation only one time with respect to any one policy.".
