3that the individual is unable to make an application in person because of the
4limitations provided under this paragraph for receiving such applications, the clerk
5shall provide the individual with the telephone numbers of the members of the
6assembly and senate who represent the individual and of the governor and instruct
7the individual to forward his or her complaint to those representatives and the
8governor.".
