AB293-ASA1-AA1,1,2 1. Page 6, line 24: before “written" insert “separate".

AB293-ASA1-AA1,1,5 2. Page 6, line 24: after “consent" insert “, independent from any other consent, permission, disclosure, or acknowledgement, in a manner consistent with applicable law,".

AB293-ASA1-AA1,1,7 3. Page 22, line 10: delete “A notary" and substitute “Except for the administration of an oath before a witness at a deposition, a notary".
