(2m) Any mental health care provider who provides mental health services
4in this state by telehealth pursuant to sub. (2) shall, within 30 days of the first day
5that the mental health care provider provides mental health services in this state by
6telehealth, submit to the department on a form prescribed by the department any
7information that the department requires to confirm that the mental health care
8provider possesses a license, registration, certificate, or other approval that qualifies
9the mental health care provider to provide mental health services in this state by
10telehealth pursuant to sub. (2).”.
