Phar 2.02 Phar 2.02 Application procedure for original licensure.
Phar 2.02(1)(1)Except as provided in s. Phar 2.06 (2), each applicant for original licensure as a pharmacist shall submit all of the following:
Phar 2.02(1)(a) (a) Completed application form with the signature of the applicant.
Phar 2.02(1)(b) (b) A statement from the dean of the school of pharmacy or the academic records office of the respective educational institution that the applicant has graduated from the pharmacy school.
Phar 2.02(1)(c) (c) If the applicant was foreign educated, evidence satisfactory to the board that the applicant has obtained certification by the foreign pharmacy graduate examination committee.
Phar 2.02(1)(d) (d) Evidence of having completed an internship in the practice of pharmacy, consisting of a minimum of 1,500 hours, which shall consist of one or more of the following:
Phar 2.02(1)(d)1. 1. A statement from the dean of the school of pharmacy or the academic records office certifying the number of hours that the applicant has successfully completed in a practical experience program of the respective educational institution.
Phar 2.02(1)(d)2. 2. A statement from a supervising pharmacist certifying the number of hours that the applicant was supervised by that supervising pharmacist in an internship in the practice of pharmacy described in ch. Phar 17.
Phar 2.02(1)(d)3. 3. Verification of practical experience acquired by the applicant in another state, which is approved and verified by the board or by the agency which is the equivalent of the board in the state in which the practical experience was acquired.
Phar 2.02(1)(e) (e) The fees required under s. 440.05 (1), Stats.
Phar 2.02(1)(f) (f) Evidence of having passed the NAPLEX.
Phar 2.02(1)(g) (g) Evidence of having passed the multi-state pharmacy jurisprudence examination with Wisconsin as primary state.
Phar 2.02 Note Note: Applications are available upon request to the board office located at 1400 East Washington Avenue, P. O. Box 8935, Madison, WI 53708.
Phar 2.02(2) (2)Any change of name made prior to admission to examination shall be supported by an affidavit satisfactory to the board.

Phar 2.05 Phar 2.05 Application procedure for persons licensed in another state. Except as provided in s. Phar 2.06 (2), each applicant licensed as a pharmacist in another state shall submit all of the following:
Phar 2.05(1) (1)Completed application form with the signature of the applicant and fee as determined by the department under s. 440.05, Stats.
Phar 2.05(2) (2)NABP Clearinghouse license transfer application.
Phar 2.05(3) (3)Evidence of having passed the multi-state pharmacy jurisprudence examination with Wisconsin as primary state.

Phar 2.06 Phar 2.06 Application procedure for service members, former service members, and their spouses.
Phar 2.06(1)(1)In this section:
Phar 2.06(1)(a) (a) “Former service member” has the meaning given in s. 440.09 (1) (a), Stats.
Phar 2.06(1)(b) (b) “Service member” has the meaning given in s. 440.09 (1) (b), Stats.
Phar 2.06(1)(c) (c) “Spouse” has the meaning given in s. 440.09 (1) (c), Stats.
Phar 2.06(2) (2)Each applicant for reciprocal licensure as a pharmacist shall submit all of the following, notwithstanding the items submitted under ss. Phar 2.02 and 2.05:
Phar 2.06(2)(a) (a) A completed application form with the signature of the applicant.
Phar 2.06(2)(b) (b) A fee as determined by the department under s. 440.05 (2), Stats.
Phar 2.06(2)(c) (c) A statement that the applicant resides in this state.
Phar 2.06(2)(d) (d) Documentation that the applicant is a service member, former service member, or the spouse of a service member or former service member. If an applicant is unable to provide the documentation, the applicant may submit an affidavit to the board stating that the applicant is a service member, former service member, or the spouse of a service member or former service member.
Phar 2.06(2)(e) (e) Evidence the applicant is in good standing with the governmental authorities in every jurisdiction outside this state that have granted the applicant a license, certification, registration, or permit that qualifies the individual to perform acts authorized under a pharmacist license granted by the board.

Phar 2.06(3) (3)A license granted under this section expires on the renewal date specified in s. 440.08 (2) (a) 55., Stats., except that if the first renewal date specified in s. 440.08 (2) (a) 55., Stats., after the date on which the license is granted is within 180 days of the date on which the license is granted, the license expires on the second renewal date specified in s. 440.08 (2) (a) 55., Stats., after the date on which the license is granted.
