AB518-ASA1-AA4,1,4 3. Page 7, line 24: delete "$50" and substitute "$20".

AB518-ASA1-AA4,1,6 4. Page 7, line 24: delete the material beginning with "If the insured" and ending with "representative." on page 8, line 2.
