AB109-AA1,1,1 At the locations indicated, amend the bill as follows:

AB109-AA1,1,3 1. Page 3, line 8: delete that line and substitute "shall notify the pupil or the pupil's parent or guardian of the impending survey or questionnaire at".

AB109-AA1,1,6 2. Page 3, line 9: after "questionnaire." insert "The notice shall include the purpose of the survey or questionnaire, information on how to review a copy of the survey or questionnaire, and a request for consent.".
