ASSEMBLY AMENDMENT 1, TO 2001 ASSEMBLY BILL 876

AB876-AA1,1,1 At the locations indicated, amend the bill as follows:

AB876-AA1,1,3 1. Page 2, line 2: after "charges," insert "requiring the commissioner of insurance to develop uniform health insurance application forms,".

AB876-AA1,1,4 2. Page 12, line 21: after that line insert:

AB876-AA1,1,5 " Section 17m. 601.41 (8) of the statutes is created to read:

AB876-AA1,1,6 601.41 (8) Uniform application form. (a) In this subsection:

AB876-AA1,1,7 1. "Group health benefit plan" has the meaning given in s. 632.745 (9).

AB876-AA1,1,8 2. "Small employer" has the meaning given in s. 635.02 (7).

AB876-AA1,1,9 3. "Small employer insurer" has the meaning given in s. 635.02 (8).

AB876-AA1,2,2 (b) The commissioner shall develop, by no later than the first day of the 7th month beginning after the effective date of this paragraph .... [revisor inserts date], a uniform application form that a small employer insurer must use when a small employer applies for coverage under a group health benefit plan offered by the small employer insurer.".

AB876-AA1,2,3 3. Page 13, line 19: after that line insert:

AB876-AA1,2,4 " Section 19m. 635.10 of the statutes is created to read:

AB876-AA1,2,9 635.10 Uniform application. Beginning no later than the first day of the 13th month beginning after the effective date of this section .... [revisor inserts date], every small employer insurer shall use the uniform application form developed by the commissioner under s. 601.41 (8) (b) when a small employer applies for coverage under a group health benefit plan offered by the small employer insurer.".
