AB772-ASA1-AA1,1,4 2. Page 3, line 4: delete "or" and substitute "or,".

AB772-ASA1-AA1,1,6 3. Page 3, line 5: after "448" insert ", or who is a nurse practitioner, as defined in s. 255.06 (1) (d),".
