66.31 Notice of regular absentee voting privilege. (1) Whenever an elector
7registers in person, except under s. 6.55 (2), the municipal clerk or special
8registration deputy shall provide to the elector written notification, on a form
9prescribed by the board, informing the elector of the privilege of requesting that
10absentee ballots be mailed to the elector regularly under s. 6.86 (2). The clerk or
11special registration deputy shall accept an application under s. 6.86 (2) from any
12elector who registers with the clerk or deputy. Each special registration deputy shall
1deliver the application to the municipal clerk together with the elector's registration
2form.

3(2) If an elector registers by mail, the municipal clerk shall, promptly upon
4entering the elector's registration on the registration list, send the elector a 1st class
5postcard containing the notice required under sub. (1).".

(4) Notice of regular absentee voting privilege. After each elector casts
9his or her ballot, an inspector shall provide to the elector written notification, on a
10form prescribed by the board, informing the elector of the privilege of requesting that
11absentee ballots be sent to the elector regularly under s. 6.86 (2). The inspector shall
12accept an application under s. 6.86 (2) from any elector who votes at the polling place
13and shall deliver the application to the municipal clerk after the close of the polls.".

(2) (a) An elector
who is indefinitely confined because of age, physical
17illness or infirmity or is disabled for an indefinite period may by
signing a statement
18to that effect written request to the municipal clerk of the municipality where the
19elector resides require that an absentee ballot be sent to the elector automatically for
20every election. The application form and instructions shall be prescribed by the
21board, and furnished upon request to any elector by each municipality. The envelope
22containing the absentee ballot shall be clearly marked as not forwardable. If any
23elector
is no longer
indefinitely confined wishes to have an absentee ballot sent to
24him or her under this subsection, the elector shall
so notify the municipal clerk
in
1writing. The clerk shall then discontinue sending absentee ballots to the elector
2under this subsection.".

(1) (c) Prescribe forms required by ss. 6.24 (3) and (4), 6.30 (4),
6.31, 6.33
6(1), 6.40 (1) (a), 6.47 (1) (a) 2. and (3), 6.55 (2) and (3),
6.80 (4), and 6.86 (2) and (3).
7All such forms shall contain a statement of the penalty applicable to false or
8fraudulent registration or voting through use of the form. Forms are not required
9to be furnished by the board.".
