6human service vehicle operated by a specialized transportation service under s.
785.21, or a transportation vehicle for elderly persons or persons with disabilities
8under this subsection.

10means a vehicle that is owned or operated by a caregiver-related entity, or operated
11by a 3rd-party under contract with a caregiver-related entity, and that meets the
12definition of human service vehicle under s. 340.01 (23g) except that it is not used
13in connection with a transportation assistance program.

(b) 1. Except as provided in subd. 2., every caregiver-related entity that owns
15or operates, or contracts for the operation of, any transportation vehicle for elderly
16persons or persons with disabilities shall issue to each operator of the vehicle an
17identification card containing the person's name and photograph and identifying the
18person as an operator providing transportation on behalf of the caregiver-related
19entity.

2. A caregiver-related entity that contracts for the operation of any
21transportation vehicle for elderly persons or persons with disabilities may, under
22such a contract, require the contractor to issue to each operator of the vehicle who
23is an employee or agent of the contractor an identification card containing all of the

21provider of specialized medical vehicle transportation or its contractor under par. (c).

2. The name, address, and telephone number of the provider of specialized
23medical vehicle transportation is prominently displayed on the specialized medical

1(3) The department shall establish guidelines for the display of information
2under subs. (1) (e) and (2) (d) 2. and (e) 2. and for the display of identification cards
3under sub. (2) (b) and (c) and s. 85.21 (3m) (b) 8. and that prescribe a standard format
4for the appearance of identification cards required to be issued under sub. (2) (b) and
5(c) and s. 85.21 (3m) (b) 8. In developing these guidelines, the department shall solicit
6input from organizations that advocate for the state's elderly persons, persons with
7disabilities, and persons who rely on transportation by specialized medical vehicles
8and shall take into account that 3rd parties may contract with multiple
9caregiver-related entities.
