AB182-ASA1-AA1,1,3 1. Page 3, line 23: delete the material beginning with ", address" and ending with "information" on line 24 and substitute "or business name".

AB182-ASA1-AA1,1,5 2. Page 3, line 24: after "product," insert "the city, state, and zip code of the person's residence, the person's telephone number or electronic mail address,".
