1An Act to amend 252.04 (3) of the statutes; relating to: including information
2about immunization waivers in enrollment or admission materials for schools,
3child care centers, and nursery schools.

Under current law, a student admitted to elementary, middle, junior, or senior high school, a child care center, or a nursery school must, within 30 days of admission, present evidence of having completed the first immunization for each vaccine required for the students grade and being on schedule for the remainder of the basic and booster immunization series for certain diseases. This immunization requirement is waived, however, if the student or the students parent, guardian, or legal custodian instead submits a written statement to the school, child care center, or nursery school objecting to the immunization for reasons of health, religion, or personal conviction. Under current law, at the time a school, child care center, or nursery school notifies a student, parent, guardian, or legal custodian of the immunization requirement, the school, child care center, or nursery school must also inform the student, parent, guardian, or legal custodian of their right to a waiver from the immunization requirement. Under this bill, a school, child care center, or nursery school must provide a procedure for presenting the waiver to the school, child care center, or nursery school with each health-related form it requires a student, parent, guardian, or legal custodian to complete before the student may be enrolled or admitted.
The people of the state of Wisconsin, represented in senate and assembly, do enact as follows:

7or legal custodian of the immunization requirements, it shall inform the person in
8writing of the persons right to a waiver under this subsection. A school, child care
9center, or nursery school shall provide a procedure for presenting the waiver to the
10school, child care center, or nursery school with each health-related form it requires
11a student, parent, guardian, or legal custodian to complete before the student may
12be enrolled or admitted.
