16(2) Failure to meet deadline. (a) Subject to sub. (4), the commissioner shall
17refund fees paid by the applicant for a certificate of authority or license specified in
18sub. (1) if the commissioner fails to provide the applicant with written notice, which
19may be by electronic mail, that the commissioner has approved or disapproved the
20application for the certificate of authority or license, including the specific facts upon
21which any disapproval is based, before the expiration of the period established under
22sub. (1) for the certificate of authority or license.

(am) Subject to sub. (4), if the commissioner fails to provide the applicant for

24application form shall require the applicant to provide the applicant's social security
25number, if the applicant is a natural person unless the applicant does not have a

2the applicant is not a natural person. The fee specified in s. 601.31 (1) (mr) shall
3accompany the application. The commissioner may not issue a license under this
4subsection unless the applicant provides his or her social security number, unless the
5applicant does not have a social security number, or its federal employer
6identification number, whichever is applicable. If the applicant is a natural person
7who does not have a social security number, the commissioner may not issue a license
8under this subsection unless the applicant provides, on a form prescribed by the
9department of workforce development, a statement made or subscribed under oath
10or affirmation that the applicant does not have a social security number.
Approval
11or disapproval of an application for a license under this subsection is subject to s.
12601.58.

25department of regulation and licensing shall submit in proposed form the rules
1required under section 440.03 (1m) of the statutes, as created by this act, for all
2credentials, as defined in section 440.01 (2) (a) of the statutes, at one time.
