1995 Wisconsin Act 27 created s.
46.27 (11g) of the statutes, which requires the Department of Health and Family Services to submit an annual report on its Community Options Program. The attached report describes the persons served, program expenditures and services delivered through the Community Options Program in calendar year 1996.
Community Options is designed to relocate or divert people who need long term care from nursing homes. This is accomplished by offering an alternative package of community services to elderly and disabled persons who are otherwise eligible for Medicaid-funded nursing home care.
The Departments allocates funds to county human service agencies to deliver these community services. In providing services, the county agency may not expend more, on average, than the state portion (about 40%) of the Medicaid cost for nursing home care. The county agency must also maximize funds by accessing any federal funds (Waiver and other Medicaid) available for an individual before using Regular Community Options funds.
The annual report examines program activity for both the Community Options Program and the Community Options Program-Waiver in calendar year 1996.

Pursuant to Chapter
109 of Wisconsin Statutes I hereby submit the department's 1997 annual report on business closings and/or mass layoffs to you. This report lists all businesses which notified the department in 1997 of planned closings/mass layoffs and complaints the department received of alleged notification violations. The report indicates the businesses involved, location, date complaint received, determination issued and disposition of each case. Beyond complaints filed in 1997, the list updates the disposition of complaints pending at the end of 1996 or filed early in 1998.
Nine 1996 plant closing complaints remained under investigation at the beginning of 1997. The department settled one violation by collecting $147,000 in penalty wage and a $30,000 surcharge. Three cases are currently at the Department of Justice pending litigation of violations. The department found no violations in four other cases and one case remains under investigations at this time.
The department received 22 complaints in 1997 and once complaint so far in 1998. The department completed 17 investigations of 1997 complaints finding violations in three cases and no violations in 14. The remaining five 1997 cases and the one 1998 case remain under investigation at this time.

RE: 1998-1999 Emergency Medical Services State Plan
Attached is the state plan for Emergency
Medical Services State (EMS) in Wisconsin for the years 1998-1999. This report meets the statutory requirement [s. 146.53(2)] that the Department shall keep the state plan current and reorder priorities based on recommendations of the EMS Board. The report summarizes EMS activity on what has been done in the past two years, what is currently in process, and what is planned or being considered for the near future.
The statute also requires a public hearing on a draft of the plan prior to final adoption and submission to the legislature under s.
13.172(2). The public hearing was held January 21, 1998.

Wisconsin school laws include the following provision in section 115.45 (6)(b) and (c):
SECTION 115.45 Grants for preschool to grade 5 programs.
(6) The state superintendent shall:
(b) By March 1, 1986, and annually thereafter, submit to the joint committee on Finance and the chief clerk of each house of the legislature, for distribution to the appropriate standing committees under s.13.172(3), a budget report detailing the grants he or she intends to award under this section in the next fiscal year. The report shall provide summary data on the results of the annual testing required under sub. (4)(b) and include a description of the guidelines used to determine the individual schools and private service providers that will receive funds under this section and the types of expenditures eligible for such funds.
(c) Annually submit to the legislature under s.
13.172 (2) a report on the academic progress made by pupils enrolled in preschool to grade 5 programs under this section.
All Preschool-to-Grade-5 (P-5) Evaluation Reports for 1996-97 are contained herein as well as a report on the academic progress for all schools funded by the P-5 program. If you have questions, please contact my office or Sandra Dercole in the DPI Office of Urban Education (414-227-4466).
