AB100-AA7,1,11 At the locations indicated, amend the bill as follows:
AB100-AA7,1,2 21. Page 2, line 2: delete "Uniform application" and substitute "Application".
AB100-AA7,1,3 32. Page 2, line 2: after " individual" insert "major medical".
AB100-AA7,1,9 43. Page 2, line 3: delete the material beginning with that line and ending with
5page 3, line 5, and substitute "(a) The commissioner shall by rule develop for use by
6insurers a uniform application for individual major medical health insurance
7policies. The commissioner shall also by rule prescribe the format for the uniform
8application, which may not exceed 9 pages in length. The commissioner shall consult
9with insurers when developing the uniform application.
AB100-AA7,2,510 (b) An insurer may use the uniform application or the insurer's own application
11for individual major medical health insurance policies. An insurer may not use an
12application other than the uniform application, unless it has been approved by the
13commissioner. A year after initial approval, the commissioner may review any

1insurer's application that the commissioner has approved for use to determine if the
2application continues to satisfy the commissioner's standards for individual health
3insurance policy applications. The commissioner may promulgate rules to ensure
4that individual health insurance policy applications are clear and easy to
5understand.
AB100-AA7,2,116 (c) The commissioner shall by rule develop a pool of health questions for the
7uniform application. Insurers using the uniform application shall use only the
8approved health questions in an application. Insurers may submit application
9questions to the commissioner to be used with the uniform application. The
10commissioner shall consult with insurers when developing the pool of questions for
11the uniform application.
AB100-AA7,2,1412 (d) The use by an insurer of an individual major medical health insurance
13policy application does not limit the ability of the insurer to request or obtain
14additional information for underwriting purposes.
AB100-AA7,2,2115 (e) The commissioner shall publish a notice in the Wisconsin Administrative
16Register stating the effective date of the rules under pars. (a), (b), and (c). If an
17insurer chooses to use the uniform application, the insurer shall notify the
18commissioner in writing. The commissioner shall promulgate rules to allow
19electronic use of the uniform application. The effective date for electronic use of the
20application may not be sooner than one year following the effective date of the
21uniform application.
AB100-AA7,2,2222 (f) This subsection does not apply to any of the following:
AB100-AA7,2,2423 1. An individual disability insurance policy that is designed to provide only
24short-term coverage.
AB100-AA7,3,2
12. A limited disability insurance policy or certificate marketed as other than
2a major or comprehensive medical policy.
AB100-AA7,3,43 3. An individually underwritten medical plan marketed or sold only to
4students.
AB100-AA7,3,55 4. Coverage that is only accident insurance.
AB100-AA7,3,76 5. Hospital indemnity insurance or fixed indemnity insurance or coverage only
7for a specified disease or illness.
AB100-AA7,3,98 6. A disability insurance policy that covers only certain specified diseases or a
9critical illness plan.
AB100-AA7,3,1110 7. Individually underwritten medical plans not marketed as major medical
11coverage.
AB100-AA7,3,1212 8. Credit health insurance.
AB100-AA7,3,1313 9. A disability insurance policy that covers only dental care.
AB100-AA7,3,1414 10. Disability income insurance.".
