SB620,6,1717 2. "Dependent" has the meaning given in s. 635.02 (3c).
SB620,6,2218 (b) Except as provided in par. (d), every disability insurance policy, and every
19self-insured health plan of the state or a county, city, town, village or school district,
20that provides coverage for a dependent of the insured shall provide coverage of
21appropriate and necessary immunizations, from birth to the age of 2 years, for a
22dependent who is a child of the insured.
SB620,6,2423 (c) The coverage required under par. (b) may not be subject to any deductibles,
24copayments or coinsurance under the policy or plan.
SB620,6,2525 (d) This subsection does not apply to any of the following:

11. A disability insurance policy that covers only certain specified diseases.
SB620,7,32 2. A health care plan offered by a limited service health organization, as defined
3in s. 609.01 (3).
SB620,7,44 3. A long-term care insurance policy, as defined in s. 600.03 (28g).
SB620,7,55 4. A medicare replacement policy, as defined in s. 600.03 (28p).
SB620,7,66 5. A medicare supplement policy, as defined in s. 600.03 (28r).
SB620, s. 17 7Section 17 . Initial applicability.
SB620,7,108 (1) The treatment of section 252.04 (5) (a) and (c) of the statutes first applies
9to students admitted to a school, day care center or nursery school during the 1996-97
10school year.
SB620,7,1311 (2)  The treatment of sections 40.51 (8) and (8m), 60.23 (25), 66.184, 111.70 (1)
12(a) and (4) (n), 111.91 (2) (k), 120.13 (2) (g), 185.981 (4t), 185.983 (1) (intro.) and
13632.895 (11) of the statutes first applies to all of the following:
SB620,7,1614 (a)  Except as provided in paragraph (b) and (c), disability insurance policies
15that are issued or renewed, and self-insured health plans that are established,
16extended, modified or renewed, on the effective date of this paragraph.
SB620,7,1917 (b)  Disability insurance policies covering employes who are affected by a
18collective bargaining agreement containing provisions inconsistent with this act
19that are issued or renewed on the earlier of the following:
SB620,7,20 201.  The day on which the collective bargaining agreement expires.
SB620,7,22 212.  The day on which the collective bargaining agreement is extended,
22modified or renewed.
SB620,7,2523 (c)  Self-insured health plans covering employes who are affected by a
24collective bargaining agreement containing provisions inconsistent with this act
25that are established, extended, modified or renewed on the earlier of the following:
SB620,8,1
11.  The day on which the collective bargaining agreement expires.
SB620,8,3 22.  The day on which the collective bargaining agreement is extended,
3modified or renewed.
SB620, s. 18 4Section 18. Effective dates. This act takes effect on the day after
5publication, except as follows:
SB620,8,96 (1) The treatment of sections 40.51 (8) and (8m), 60.23 (25), 66.184, 111.70 (1)
7(a) and (4) (n), 111.91 (2) (k), 120.13 (2) (g), 185.981 (4t), 185.983 (1) (intro.) and
8632.895 (11) of the statutes and Section 17 (2 ) of this act take effect on the first day
9of the 5th month beginning after publication.
