5deaf or severely hearing impaired by a physician or by an audiologist licensed under
6subch. II of ch. 459.
AB1217,4,97 (c) The coverage required under par. (b) may be subject to any limitations,
8exclusions, or cost-sharing provisions that apply generally under the disability
9insurance policy or self-insured health plan.
AB1217,4,1010 (d) This subsection does not apply to any of the following:
AB1217,4,1111 1. A disability insurance policy that covers only certain specified diseases.
AB1217,4,1412 2. A health care plan offered by a limited service health organization, as defined
13in s. 609.01 (3), or by a preferred provider plan, as defined in s. 609.01 (4), that is not
14a defined network plan, as defined in s. 609.01 (1b).
AB1217,4,1515 3. A long-term care insurance policy.
AB1217,4,1616 4. A medicare replacement policy or a medicare supplement policy.
AB1217, s. 10 17Section 10. Initial applicability.
AB1217,4,1818 (1) This act first applies to all of the following:
AB1217,4,2119 (a) Except as provided in paragraphs (b) and (c ), disability insurance policies
20that are issued or renewed, and self-insured health plans that are established,
21extended, modified, or renewed, on the effective date of this paragraph.
AB1217,4,2422 (b) Disability insurance policies covering employees who are affected by a
23collective bargaining agreement containing provisions inconsistent with this act
24that are issued or renewed on the earlier of the following:
AB1217,4,25 251. The day on which the collective bargaining agreement expires.
AB1217,5,2
12. The day on which the collective bargaining agreement is extended, modified,
2or renewed.
AB1217,5,53 (c) Self-insured health plans covering employees who are affected by a
4collective bargaining agreement containing provisions inconsistent with this act
5that are established, extended, modified, or renewed on the earlier of the following:
AB1217,5,6 61. The day on which the collective bargaining agreement expires.
AB1217,5,8 72. The day on which the collective bargaining agreement is extended, modified,
8or renewed.
AB1217, s. 11 9Section 11. Effective date.
AB1217,5,1110 (1) This act takes effect on the first day of the 7th month beginning after
11publication.
