(a) 3. and (d) 4., no adjusted premium shall be deemed considered to exceed 4 per cent % of the amount of insurance or uniform amount equivalent thereto. The date of issue of a policy for the purpose of this subsection and sub. (5) shall be the date as of which the rated age of the insured is determined.
