(3) Any person who observes a violation of s. 346.67 to 346.69 may prepare a
4written report indicating that a violation has occurred. If possible, the report shall
5contain the following information:

(a) The time and the approximate location at which the violation occurred.

(b) The license number and color of the motor vehicle involved in the violation.

(c) Identification of the motor vehicle as an automobile, station wagon, motor
9truck, motor bus, motorcycle or other type of vehicle.

(d) A description of the accident, including a description of the person, vehicle
11or property that was struck.

12(3m) (a) Within 24 hours after observing the violation, a person may deliver
13a report prepared under sub. (3) to a traffic officer of the county or municipality in
1which the violation occurred. A report that does not contain all of the information
2in sub. (3) shall nevertheless be maintained by the county or municipality for
3statistical purposes.
