In accordance with Sec. 1.11 (2)(j) Wis. Stats., the Department of Health and Family Services reports that one Environmental Assessment (EA) was conducted by the Department of Health and Family Services during the reporting period July 1, 1999, through June 30, 2000.
The EA was done for the project to construct a 15-bed addition to the Mendota Juvenile Treatment Center at Mendota Mental Health Institute in Madison, Wisconsin. It was determined that the construction of this addition does not have a significant negative effect on the environment.

Accordingly, the agency was not required to prepare any environmental assessments or impact statements under s.
1.11.

"Annually, no later than September 15, submit a report to the chief clerk of each house of the legislature for distribution to the legislature under s.
13.172 (2), including the number of proposed actions for which the agency conducted an assessment of whether an impact statement was required under par. (c) and the number of impact statements prepared under par. (c)."
This report addresses the activities of the Department of Transportation for Fiscal Year 2000, the period from July 1, 1999 to June 30, 2000.

46.014 (4) of the Wisc. Stats., the Department is required to submit an annual report to the Legislature concerning the activities of the community action agencies under s.
46.30, and their effectiveness in promoting social and economic opportunities for poor persons.
The attached report describes the use of the Community Services Block Grant funding, and the services and activities of the community action agencies carried out to reduce poverty, revitalize low-income communities, and empower low-income individuals and families to become fully self-sufficient.
