UWS 23.05(1)(a) (a) Evidence pertaining to dates of accredited medical or dental school attendance and graduation.
UWS 23.05(1)(b) (b) Evidence of educational loan indebtedness and education loan outstanding balance.

UWS 23.06 UWS 23.06Breach of agreement.
UWS 23.06(1) (1) The following actions, if taken by a physician or dentist who has entered into an agreement with the Board of Regents under the state physician and dentist loan repayment program for repayment of loans entirely from state funds, after July 1, 1998, shall constitute a breach of that agreement:
UWS 23.06(1)(a) (a) No longer practicing at an eligible clinic.
UWS 23.06(1)(b) (b) Reducing hours below the minimum required by the Board of Regents.
UWS 23.06(1)(c) (c) Failing to treat medicare or medicaid patients.
UWS 23.06(1)(d) (d) Failing to apply funds received from the Board of Regents to repayment of eligible loans.
UWS 23.06(1)(e) (e) Failing to timely supply documentation required by the Board of Regents.
UWS 23.06(1)(f) (f) Failing to work at least 45 weeks per year.
UWS 23.06(1)(g) (g) Failing to meet the additional requirements outlined for psychiatrists in s. UWS 23.03 and dentists in s. UWS 23.035.
UWS 23.06(2) (2) The following actions, if taken by a physician or dentist who has entered into an agreement with the Board of Regents under the expanded loan assistance program under s. 36.60 (9), Stats., shall constitute a breach of that agreement:
UWS 23.06(2)(a) (a) No longer practicing at a public or private, nonprofit entity.
UWS 23.06(2)(b) (b) Reducing hours below the minimum required by the Board of Regents.
UWS 23.06(2)(c) (c) Failing to take medicare assignment.
UWS 23.06(2)(d) (d) Failing to treat medicaid patients.
UWS 23.06(2)(e) (e) Failing to apply funds received from the Board of Regents to repayment of eligible loans.
UWS 23.06(2)(f) (f) Failing to timely supply documentation required by the Board of Regents.
UWS 23.06(2)(g) (g) Failing to work at least 45 weeks per year.
UWS 23.06(2)(h) (h) Failing to use a sliding fee scale or comparable method of determining payment arrangements for patients who are not eligible for medicare or medical assistance and who are unable to pay the customary fee for the physician's or dentist's services. Sliding fee scales may vary from clinic to clinic but, at a minimum, shall address persons with incomes below 200% of the federal poverty level.

UWS 23.07 UWS 23.07Penalties for breach of agreement.
UWS 23.07(1)(1) A physician or dentist who breaches an agreement with the Board of Regents in the first year of the physician's or dentist's obligation shall repay to the Board of Regents the amount already received plus an amount equal to the total months of obligation multiplied by $1,000.
UWS 23.07(2) (2) A physician or dentist who breaches an agreement with the Board of Regents in the second year of the physician's or dentist's obligation shall repay to the Board of Regents the amount already received plus an amount equal to the number of months remaining in the agreement multiplied by $1,000.
UWS 23.07(3) (3) A physician or dentist who breaches an agreement with the Board of Regents in the third year of the physician's or dentist's obligation shall repay to the Board of Regents the amount already received for the months remaining in the agreement plus an amount equal to the number of months remaining in the agreement multiplied by $1,000.

UWS 23.08 UWS 23.08Waiver of penalties. The Board of Regents may waive any penalty for breach of agreement if the physician or dentist is unable to serve or repay due to a permanent physical or mental impairment that prevents the physician or dentist from working in the physician's or dentist's profession. The physician or dentist shall provide verification of his or her condition from an appropriate board certified specialist or specialists that will reasonably convince the Board of Regents that the physician or dentist will be unable to continue working in the physician's or dentist's profession.

UWS 23.09 UWS 23.09Suspension. A physician's or dentist's contract may be suspended by the Board of Regents, without penalty, for a period of time agreed upon by the physician and the Board of Regents for certain hardships, including an extended illness or family leave that exceeds the maximum of 7 weeks allotted each year, or termination of employment that requires the physician or dentist to seek employment in another eligible practice area.
